RISE EDUCATION & CARE GROUP · DAILY LOG
Keeping young people, home and RISE connected.
A daily communication log for a young person, RISE staff, home and the people who support them. Sleep, mood, medication, food, triggers, academic progress and wellbeing tracking.
Who it's for
Student
See your own day and tell us how you are feeling.
Parent or carer
Share how things are going at home and read what's happened with RISE staff.
RISE staff
Write daily notes, recording food, triggers, academic progress and wellbeing tracking.
Visiting professional
View a student's record. You cannot change anything.
Private by design
- Everyone signs in as themselves. You only ever see the young people you are connected to.
- Visiting professionals can read a record to understand a student, but can never change it.
- Students can see their own day and add how they are feeling, in their own words or with a tap.
- Health information is treated as special category data and is never shared beyond the people linked to that student.
Written to be trauma informed
Nothing here is compulsory. There are no red warnings, no scores and no streaks. Language is curious rather than judgemental — we ask what happened and what might have led up to it, not what someone did wrong. Every section can be left blank, and a young person is always described as a person first.
